Description

 Essential Functions:

Reasonable accommodations may be needed to enable individuals with disabilities to perform the essential functions.

1. Completes at least an hour of education each month per CMS regulations and tracks it on their competency sheet.
2. Functions within the limits of LNA role and complies with all state & regulatory rules
3. Follows the patient care plan including personnel care, housekeeping, errands, etc.
4. Informs the primary clinician / Case Manager of changes in patient condition, as appropriate and documents interdisciplinary communications.
5. Ensures that clinical competencies are completed prior to providing standard and specialized care and that competency sheet and/or patient record demonstrates.
6. Documents patient care per agency protocol and CMS regulations.
7. Adheres to Agency dress code, presenting a professional image.
8. Collaborates with the interdisciplinary team
9. Adheres to Agency handwashing technique, infection control and bag technique.
10. Is an active participant in Case Management and agency in-services.
