Essential Functions:
Reasonable accommodations may be needed to enable individuals with disabilities to perform the essential functions.
- Completes at least an hour of education each month per CMS regulations and tracks it on their competency sheet.
- Functions within the limits of LNA role and complies with all state & regulatory rules
- Follows the patient care plan including personnel care, housekeeping, errands, etc.
- Informs the primary clinician / Case Manager of changes in patient condition, as appropriate and documents interdisciplinary communications.
- Ensures that clinical competencies are completed prior to providing standard and specialized care and that competency sheet and/or patient record demonstrates.
- Documents patient care per agency protocol and CMS regulations.
- Adheres to Agency dress code, presenting a professional image.
- Collaborates with the interdisciplinary team
- Adheres to Agency handwashing technique, infection control and bag technique.
- Is an active participant in Case Management and agency in-services.
